I've got one and only one customer getting the following exception - here's
the madexcept stack trace ...

So, the issue is not reproducible for you ?  Is it reproducible at the
customer's site ?

Tim,

Unfortunately the files are huge and I cannot get his data files via email.
I had him run a repair on the data files and
all his data was subsequently lost after the repair.  I have never seen this
error before but it must have been due to
some type of corruption in the table

Yes, that is the most likely case, especially if the repair ended up
removing most of the rows because they had an issue with a primary key
constraint, etc.
