I am getting the following error message when attempting to insert records into a table in DBISAM:

[ADO NET Destination [468]] Error: An exception has occurred during data insertion, the message returned from the provider is: ERROR [HY000] [Elevate Software][DBISAM] DBISAM Engine Error # 9729 Duplicate key found in the index 'Code' of the table 'insuranceplan'

I have verified that no duplicate values are going into the code field.  Any ideas?

Nevermind.  I found the stupid thing.  Hidden in plain sight.
