Here's the ElevateDB help manager unit, along with the compilation code.
Please see the ElevateDB Manager source code for how the help manager is
used with a customized IE web browser for display and navigation.

The compilation code is incomplete, but basically the process revolves
around generating the HTML files in the same way that you would do for HTML
help, populating some tables with the HTML and image files, as well as a the
index and table of contents, and then writing them all out as a single,
compressed file.
