Regular index is 1D, selecting range from A to Z. 2D index can select square subrange [A..B][C..D]. This is if you want to locate an object within a given rectangle. You can't do that (fast) using regular indexes. A spatial database is a system that integrates the possiblity to add indices that can be query on a "range base" and with approximation. Mostly it's used in GIS applications to e.g. find all database entries that are located in a certain area "approximately". It also includes the possibility to calculate a "distance" between two records, again this is mostly used for GIS info systems.
